How to determine coordinates on a pre-printed form

Hi...

I am a novice C# programmer. I have a two page pre-printed form in .bmp
format. I would like to write a program to collect data from the user, and
use the data to fill in the form and then print it. What is the best way to
determine the coordinates of the text fields (to fill in) in the form to
print?

Your help in answering my question would be highly appreciated.
