If I check the do not send email option, does this automatically prevent me
sending email to the contact ?

Apart from its informational purposes, I believe it is only effectively
used in conjunction with List Builder exports; i.e. Contacts with that
checked are not sent to List Builder. You could experiment to get a
more definitive answer.

This is a field value only. I have been able to send emails to Business
Contacts eventhough it was checked not to do so.
