desktop alert displays different message

I am getting SPAM emails that are displaying messages in the desktop
alert funtion of Outlook 2003. When you open the email the message is
different. How are they doing this?

They're sending you a multipart message, with one part in plain text (shown
by the Alert) and one in HTML (seen when you open the message). The two
parts don't have to contain the same information.
