The derived folder is not available to load when I publish to my host
server's ftp site and consequently the navigation buttons do not load
properly. They work but do not have the appearance of a regular navigational
bar. How or where can I find the _derived folder and load it ?

You could try using FrontPage to publish to your site:
Open the web, then File->Publish Web (Site in FP2003)
In the destination type in ftp://mysite.com The URL must start with ftp://,
the rest will be the same as you use in your FTP client.

Publishing with FrontPage will ensure that the _derived folder and its
content will be uploaded. However, not all FTP sites can be published to
with FP, it's a case of try it and see.

Some FTP clients will not display hidden files or folders in the PC window,
unless Windows is set to also display hidden files and folders.
Open Windows Explorer, then Tools->Folder Options - View tab
Select "Show hidden files and folders"
Click OK
