Deleted messages bypassing Deleted Items folder?

We are using Outlook 2003 with an Exchange server.

I have one user who claims that very occasionally, a message that she
deletes from her inbox will apparently vanish into the ether (this may happen
to any message; there's no pattern to subject, sender, etc.) She happened to
notice this behavior on those few occasions because she would immediately
realize she shouldn't have deleted it, goes to her Deleted Items folder, and
can't find it. So there's no real time delay involved.

I have checked for any unusual settings and found none, nor does she have
any rules in place. We're not concerned about getting these deleted items
back; we just want to know why they seem to be bypassing the Deleted Items
folder.

I find it doubtful that she's "hard deleting" the items, because when you do
that you're prompted whether you really want to do that or not.

Has anyone else ever seen this occur?
