How to delete form published in Mailbox?

I'm using Outlook 2003 connecting to a Exchange. I created a custom form and
published it in the Mailbox folder (i.e. the top folder that's listed as
"Mailbox - John Smith"). How do I delete a form from there? The Form Manager
doesn't show the form. However, it is still available for me to choose under
Tools->Forms->Choose Forms.

Where is it stored and how do I remove it?
