When I am in the Address Book, there is a drop-down box on the right that is
labelled "Show Names from the :" and there is a drop-down list that has the
following contact folders:

Outlook Address Book
Contacts
SML 080512
SML Distribution
SML Newsletter
SML Newsletter
SML NL
Personal Address Book



Of the five SML entries, four are empty. I can't for the life of me
remember how I created and loaded addresses in the one that does have
entries.

These aren't distribution lists - they have distribution lists in them.
How do I get rid of the four entries that are empty? I can't find any place
to select and delete one of these folders.

Those are other contact-type folders. The Outlook Address Book shows
contact-type folders as address books.

You don't mention your Outlook version. For OL 2002/3, in Outlook's main
window click Tools>E-mail Accounts>?View or change existing directories or
address books>Next. Select "Outlook Address Book" and click Change. Remove
the empty folders there.

Consider as well removiong the Personal Address Book service. The PAB has
been obsolete for 10 years and Outlook 2007 can't use the service (although
it can import from a PAB).
