You don't mention your Outlook version. For OL 2002/3, in Outlook's main
window click Tools>E-mail Accounts>?View or change existing directories or
address books>Next. Select "Outlook Address Book" and click Change. Remove
the empty folders there.
Consider as well removiong the Personal Address Book service. The PAB has
been obsolete for 10 years and Outlook 2007 can't use the service (although
it can import from a PAB).
