Everytime I start PowerPoint it starts with a default layout with two text
boxes. The first thing I have to do is delete "click to add a title" and
"click to add a subtitle" boxes. How do I set it to default to a totally
blank slide?

Those are the placeholders. Don't delete them but use them! If you format
the placeholders in the masterslide, they will make formatting of your
slides easier.
