Data not displayed after emailing form

I designed a new Outlook form based on an existing form. Two of the fields do
not print or display the data after the form is emailed. One box is for a
number that is entered by the user. The other box is supposed to display the
user's selection from a pull down list. The same boxes work fine on the
original form.
Any suggestion? Thanks!
