Data fields empty when reading.....

I created a form, which accepts the data however, when it's sent, the data is
not there. The data fields are a simple "CheckBox"' and "TextBox". Why would
this happen & more importantly, how do I resolve it. Thanks.

Those don't look like data field names. They look like control names.
There's a big difference. Only data fields can store data so it can be saved
or transmitted. These pages should help:

That worked... thanks. Next question: In my form I have the To: with my name
in it so the form comes to me when it's filed out. Now, how do I send it to
those I want to get it? I should really go buy your book!

Is this a message form? Explain what kind of mail environment you're working
in and what this form is supposed to accomplish.
