Is it possible to create a data entry form in outlook (2003 or 2007) where
the user could fill-in fields... and then send it back to the originator?

If so, how? What is it called in outlook so I can look it up further?

You can use Custom Forms for this or use InfoPath if you have that installed
for your users as well.
