data became blank in custom form of outlook 2003

Hi, we are having a customer form published in organisational forms library,
we are having a work flow between Associate, and Analyst; first Associate
will start the work flow, fills the form and sends it to SA, SA checks it and
adds his information and either approves or disapproves and sends back to
Associate, at this point of time i.e when Associate receives it from SA, data
becomes blank, and it is happening only in outlook 2003, I'm not able to
understand why?, because it is working perfectly in previous versions of
Outlook.

version of outlook is : Microsoft Outlook 2003
Operating System: Windows XP

Does the form have code behind it? In what folder do the items using it
reside? Is the same form used for all steps in the workflow?

Hi Sue, thanks for responding,
Yes, it has code behind it, the items using it reside in normal inbox
folder, and it is using same form in all steps in the work flow, I have
checked this.
