Outlook needs to be open to use the template as its an outlook template.
while double clicking it will open outlook, its usually easier to make sure
you are logged in using an acct that had admin rights - including the
ability to create PF.
If the acct you are logged into outlook has permission to create PF, the
option will not be grayed out and you can make sure the permissions are
correct for the folder. You also need to be logged into outlook with an acct
admin rights to edit the form, otherwise any user could make changes.
