When I add command icons to the toolbars (e.g. Empty Deleted Items or Add
Sender to Blocked Senders List) they don't get saved when I exit Outlook. Am
I missing something?

outlook is not closing properly - if you are using a pda, disconnect it
first and close any other applications using outlook before closing it.
