Hi,

How can I add more controls (text box and check box) to customize the
message 'Flag for Follow Up' dialog box ?

Later, when I re-read this message, I'd like to see this dialog box opened
immediately; any one knows how to do it ?

That dialog is private to Outlook and cannot be customized.

As for displaying it, you can execute any toolbar or menu command with CommandBars techniques. See http://www.outlookcode.com/d/tips/commandbarfun.htm
