Customer Survey on web page?

Hi All: Using FP 2003, Windows XP, Access XP Pro.
Looking to provide a form page on my website to be used as a customer
service survey. I no I can have the form send data to email but would like
to set up to use access to evaluate the results and print reports based on
entries. Any ideas or templates? I do not have the advanced hosting
applications by server. www.hostsave.com

Your site must be hosted on a Windows IIS web server and your host must allow you to run ASP and use
Access, then you can use the FP DB component or search for a ASP based survey application.

Looks like his host is not a Windows host and doesn't support ASP or Access
He will have to search for a PHP / MySQL survey script

You could collect the data in CSV format and import it into Access manually.
