Custom Outlook Form is not visible to reader????

Hi - I have created a custom message form in Outlook 2007. I have published
it to a company wide folder, however, when filled out and sent to myself, it
does not pull up the form --> only a blank email.

When I am in design mode and click 'run this code' it works beautifully, but
not when published.

Any ideas? Many thanks!

Try RTF. RTF includes the infamous winmail.dat TNEF envelope that you need
for custom forms.
