custom message form recieved without custom fields - outlook 2000

I have created a message form in outlook for helpdesk requests. Thi
form is based on a standard message, however, has a smaller mai
message area and added custom controls.
The form looks perfect while creating, however, when it is recieved th
read screen looks exactly like a normal message. I have looked at th
full header etc on the received email cant see anything related to th
new fields.
All controls are bound to user-defined fields and ticked as 'visible
and 'enabled'. I have tried the form with separate read layou
disabled. And then with it enabled and with the edit read pag
identical to the edit compose page.
Nothing seems to make any difference. Even with 'send form definitio
with item', the whole received email is only 2kb in size. (The size o
the oft file is 28Kb.) I have created and published the form every wa
I can see and it doesnt work for me. This is the first time I hav
tried customised forms so it is probably something I am missing.
OFT is attached.
Please help.......

Did you set the message format to rich-text and double-click your address to
make sure it's set for rich-text? See
