I had been able to hit "ctrl a" to choose the entire document. Now it
doesn't work. How can I get this back?

Presumably something (an add-in, perhaps) removed or changed the shortcut
assignment. To put it back, use the same procedure you would use to create a
brand-new assignment:
http://www.word.mvps.org/FAQs/Custom...roToHotkey.htm. The
command you want is in the Edit group, with the name EditSelectAll.
