Cross-reference update and formatting

1) I'm working with a document that used bookmarks and hyperlinks in a table
of contents. Of course, the page number wouldn't change in the TOC when the
linked item moved in the document. I inserted a cross-reference link into
the table of contents but the page number won't automatically update when the
bookmarked item moves. I can change the page number/link by clicking F9. Is
there a way to have the page number update automatically?

2) Is there any way to make cross-references behave like hyperlinked text
(is underlined and changes colour after being clicked.)
