creating a summarized form

We have a excel file with multiple Names at the same Address.
We want to create a form that Shows the 1st name and the Address at the top
and then list just the additional names in the 'body':
for example:
john smith
xyz company
123 any street
city, st zip

qty name
1 2nd name
1 3rd name
2 4th name - and so on

Can this be done using word 2003?

Copy and paste the data into a Word document?
