Creating a New Form in Outlook 2007

I need to create a new form in Outlook 2007 for the purposes of reporting by
email NNTP access statistics on a regular basis to my ISP.
I've looked at Help and can't seem to find any way of doing this.

Is this possible?

Depends, what modifications are you trying to make?
To go to form design choose;
Tools-> Forms-> Design a Form

OK thanks, - I've seen that (why didn't Help come up with that I wonder? I
asked it how to create a new form......). A further question - I want to
create a form from a received email. How do I do that?

Open the Item-> Developer tab-> Design this Form

You can enable the Developer tab by clicking the Office logo in the left to
corner and choose Editor Options...
