I'm trying to create an e-mail template to send to people, with fields so I
can enter the date, person's name, an amount of $, etc, but the rest of the
e-mail stays the same. I can create a template but I need to know how to
have a template WITH form fields. I am using the Outlook 2003. Thank you!

Why not use a mail merge instead?
Then you can fully automate the creation and sending of the emails with the
correct information.

I do use that for other things, but for this specific form it will only be
going to 1 person at a time generally. It basically says "You have been
approved for _______, you have $______ left for 2007". So we generate the
e-mail in response to a specific request, but it is the same form e-mail
every time.

Alright, well form fields are not supported in an email. You can use Text
Boxes though when you have Outlook 2007 or Word set as your email editor.
Insert-> Text Box

When I have Word set as my e-mail editor I am able to add text boxes, but it
won't let me save the e-mail as a template, unless I'm doing something wrong?
So if I get upgraded to 2007, I will be able to save a template with text
boxes? Thanks for your help...

Compose the email like you want to and then press the Close button. It will
ask to save your changes/draft and it will end up in the Drafts folder.

Outlook 2007 will allow you to do that as well but there is no need to use
Word as the email editor for this since Outlook 2007 has a new email editor
and only a single one instead of 2.
