How to create a "rate this page"

Hello, I have a webpage (frontpage) that I would like to create a
webpage rating for similar to this one's "rate this listing":

I would also like it to show the rating at the top of all the people
that rated the page...
Can anyone give me a tips?

Would require a MySQL database or an Access database using asp or asp.net, depending upon
what your host supports.
