How to create a questionnaire

I want to be able to create a questionnaire with frontpage 2003
everytime I click on next on the screen it goes to a new screen if I cannot
do this
then in a form how can I put a next and go to a new screen.

If the answer is negative then I want a pop screen to appear.

Is this all possible

Could you explain a little better, in particular:
"every time I click on next" (what is that what do you mean) and "I want a
pop screen to appear" (this would be clear if I understood the first part).

If you have a page on the internet with your form give the URL to it also.

I am creating the page. Basicaly what I would like to do is. Create a
questionnaire for install of equipment. When they answer a question is there
is additionnal info I would like to have a popup box with this info. To
proceed to the next question they would have to click on the next button this
will bring them to the next question on a new screen. Once this is completed
they would have a review screen then at the end they would click on submit
and it would come to my email account.

I know I can create a form but I would like to have in there popup windows
and have a question on each screen instead of having it all in one window
