I would like to create a form on my website that viewers can complete and
submit back to me via e-mail. The results should look like the form.

Hi Stressed,

You may create the form and have your visitors complete it however what is emailed back to you will only have the field name and the
value the visitor entered. Note: For the preceding to work you'll have to have the FrontPage Server Extensions installed on the
server.

You can not modify or control the Email form output using the std FP forms handler
- will require hand coding and server side scripting

Insert > Form.....

But the results will come back to you as a plain email. Frontpage can't
format the email in the way you describe.
