Create a fax cover page from contacts

I would like to be able to perform a similar action as "choose a contact ->
select the actions menu item -> New Letter to Contact" except I would like to
create a fax cover sheet instead of the letter. Therefore I need to change
the template from letter wizard to fax wizard. Can this be done? Or could I
create a form that you simply choose the contact and then all of the "fax
cover fields" are self populated?
