Create database error

I creating a survey using the form wizard.

Once I finish creating the page I tried to create the database and I get the following error:

Cannot open "wpf8exwn.mdb": no such file or folder

then

Unable to upload new database

It looks like the folder where the wizard is supposed to write the database doesn't exist.

Any help would be appreciate.
