I would like to creat a rule for messages I send with a specific subject line
that when I send the message, I amsenta reminder or a task is added reminding
me to follow up on that email. Is the best way to do that through a "custom
action"? If so, how do i create a custome action?

You need to use macros and Application_ItemSend event. You can find many
samples at outlookcode.com.
