When I use the Connection Wizard to establish an internet
connection I can get through the first two steps, but not
the third.
My choices:
1) "Connect to the Internet"
2) "Set up my connection manually"
3) This is my problem - I only have one choice and that
is "Connect using broad band connection that is always
on" I can't choose "Connect using a dial-up modem"

Any ideas?

up modem but actually I'm running into the same problem my
self but I am trying to set up PPPoE and my only option
is 'Connect using broad band connection that is always on'
and I need to set up a manual connection. Can anyone help
me?

Is the option greyed out? Please make sure that the "Remote Access
Connection Manager" service is running. Go to start ->Run ->Services.msc and
start this service if it is not already running. If you face problems
starting this service, refer to KB article 329441.
http://support.microsoft.com/default.aspx?kbid=329441.

Thanks
