Can I create a form to be completed offline and select to
submit the results by email (html) so the user simply
needs to connect to the internet on completion of the
form to send me the results?

Thanks,

Tom

No

You could buy Adobe which allows for PDF form creation / data entry by the user and then they can email you the completed pdf file
as a attachment
