There is a form on: http://www.njsc-online.com/registrationform.htm. It was
working in the past, but it is now not producing the confirmation page I
set-up. I checked my end, and all seems well, so I recalculated hyperlinks
and republished. The problem persists. This is the error message:

FrontPage Error.
User: please report details to this site's webmaster.
Webmaster: please see the server's application event log for more details.

I have contacted our host and am waiting for a reply, but since this states
it is a FP error, I thought there might be something I could/should do.

The host has to review the server application log to see what the problem is.

Open the live/remote site directly in FP, then run Tools | Recalculate Hyperlinks.

Thank you, Thomas. I will do that. I do have a follow-up question. I
remembered that there was another form on another page, so I checked. That
was does still work, producing a confirmation page. Does this information
make any difference in temrs of trouble shooting the problem?

Thomas, I opened the live site directly in FP and recalculated hyperlinks.
Unfortunately, it did not solve the problem.

The best I can suggest is to compare the form properties between the two.

Yeah. I started by checking to see if anything on the form had somehow
changed, but everything looks as it should.

Thomas, I had to keep trying. So, I changed the type of file that the data
was sent to, and I also reselected the name of the confirmation page from the
directory (just in case). One of those two things did it, because it works
now. Can you offer any insight as to why this might have happened? It would
help me troubleshoot in the future.

Was that the file type you were writing to or the format for sending the email?

If email, HTML is not support via the FP Form Handler.
