Clicking submit button causes page not found, no matter configur

I have configured the submit button to save to the default file and via
e-mail. The result is always that a "page not found" screen appears. The
server is running Front Page Server and that checks out o.k.

Why are you changing the submit button? If using the FP Form Handler then you would use the Form
Properties.
