I think you've tried this first suggestion, but...
Try clicking or double-clicking or typing the first letter in the "name"
field. If autocomplete gives you a dropdown box with suggested usernames,
select the correct name (just put the pointer on it) and hit the delete key.
Then delete any cookies from the affected site.
Close IE. Open IE.
And you tried clearing all passwords from IE> Tools> Options> Content>
Autocomplete> Clear
Passwords, right?
Protected Storage PassView from
http://www.webattack.com/get/pspv.shtml will
let you delete selected username/password combos.
Finally... using registry editor to export (just to have it backed up) then
delete this registry key:
[HKEY_CURRENT_USER\Software\Microsoft\ Internet Explorer\ IntelliForms\SPW]
should clear all username/password data. Delete only the SPW key- not the
IntelliForms key.
