Checkbox not appearing on sent document

I am using Outlook 2003 in XP and created a form with several checkboxes on
it. I can check the items necessary on the new form and they are displayes,
but the form does not show the checks on the sent document. The compose and
read pages are not separated. Properties shows Visible = 1. How do I get
then to display on the sent/received item?
