check box not showing

I created a template in word 2003 that has a check box form field. I check
it and can see it in print preview, but when I receive the email the check
box is empty. Any ideas would be great. THanks

Did you remember to protect the document (this makes the check boxes and
other form elements active)? For more on document protection, see
http://word.mvps.org/faqs/customizat...nTheBlanks.htm.

Also note that you can set a password as you protect the document. That way,
no recipient can make any accidental changes to it.

Yes, I did protect the document. I have 3 form fields, one of which is a
check box. All 3 fields will print out and view fine, but when I email the
document the check box is blank and the other two fields are filled in.

And you are sure that you have been using form fields, as opposed to ActiveX
controls (found on a different toolbar), which will force Word to start in
Design mode (as a result of macro security settings)?
