Changing the Form Confirmation Page

I have set up a Form, to send to an email address. It works fine, however the
Form Confirmation page could look a little better. It lists, for example, C1
City, C2 State, C3 Zip Code...etc.

Is a Form Confirmation page mandatory? Can this be turned off? If not, how
can i not show the C1, C2, etc?

Also, when the email comes to me, the email has a title that I want to
change. Where can I change my original email Subject Title? Thanks.

You can make the form "Confirmation page" any page you want it to be.
You can also add the email subject.
Both are done by right clicking the form, and selecting "Form properties" and then clicking options.

You can name the individual fields as need by right clicking on each.
