Are you sure you were able to add it?
Turn your macro security to medium -- Tools|Macro|Security -- then install
the add-in.
At
http://skp.mvps.org/setlayout.htm it says in Instructions that a
"defaults" combo box will appear on your Standard toolbar. There's also a
screenshot of it so you can see what you're looking for.
Perhaps you can't see all the icons on your Standard toolbar? If that's the
case, either click the 3 dots at the end of the toolbar so you can see all
the icons or set the toolbars to show on 2 rows. That's under
Tools|Customize|Options, show toolbars on two rows.
