how do I find which table /record is doing this - The changes you
requested to the table were not successful because they would create
duplicate values in the index, primary key, or relationship

My db design is as follows - there is a project table, a case sub table
and in turn the case sub table as a deliverables sub table.

My form has two subforms. It was working fine till yesterday. Today it
is giving me this issue.

I search for duplicate case ID's and did not find anything.

Please advise.

You will not find duplicates as it is not allowing them. You must have been
either adding or editing a record that would result in duplicate.
