I recently created a form from scratch and while i am able to modify and send
the form the changes that i make to the check boxes and the Text boxes do not
come through to the inbox. Any ideas to why this would be?

Most likely, the controls are not bound to the Outlook properties that should be storing the data in the message.

FYI, there is a newsgroup specifically for Outlook forms issues "down the hall" at microsoft.public.outlook.program_forms or, via web interface, at http://www.microsoft.com/office/comm....program_forms
