Changes to form not showing up

I downloaded the survey template. I published the template as SurveyTemplate
to the Organization Forms library. To test the form, I made a simple change -
I changed the header from "Survey Questions" to "Service Call". I changed
the version of the form and Publish As (using the same name for the form).
Forms Manager shows it with the new version.

I open the form to send it to a customer. The new version of the form opens.
I create a couple of survey questions and send the form. The old version
shows up in my 'sent items' and in the customer's 'inbox'.

I have tried clearing the cache. I have your book but I don't seem to be
able to find why the form is not updating.
I have a feeling it's something simple ..........

Does the customer have access to your Organizational Forms library? If not,
then that form won't work. A better approach would be to create a new
message form from scratch, save it as an .oft template file, and then send
that .oft file to the customer, who can run it and send the resulting
message back to you.
