It seems as if the table of contents is picking up outline levels (Paragraph
dialog box) in addition to heading styles. To fix that, press Alt+F9 to show
field codes. The TOC field looks something like this: { TOC \o "1-3" \h
\u }. Delete the \u switch and update the TOC.
The reason for the above issue is usually the Document Map. For more, see
