I have install a stand-alone certificate authority
successfully, however every time I try requesting a new
certificate I get:

Failed to create 'Certificate.Authority.Request' object

Do you have any suggestions?

Backup the authority certificate, uninstall it, reinstall it, restore the
certificate from the backup. The easiest way.
