I am manually setting my cell heights tight, setting single lines at 13
(17 pixels high), double lines at 27 (36 pixels), and yet it seems I
have to continually re-set them. This happens when I open and close the
file, and--while I'm not positive of this--it seems to happen even if I
page forward and back through the file.

Is this just a known issue with Excel (2003, SP1) that you live with?

I've never seen any problem with xl2003 (including sp1).

Any chance you have event macros that are autofitting the rows?
