A couple of years ago I made an entry form to collect entry data. The
data was saved to the default folder (../_private). The website was
turned over to another person. They've contacted me because when
trying to save the data the user is not presented with a
username/password form that says:

A username and password are being requested by [the URL to the domain
name]. The site says: ":"

It appears that it's wanting authentication to access ../_private.
Does anyone know how to fix this?

That should read "They've contacted me because when trying to save the
data the user is *NOW* presented with a username/password form . . .

Provide a URL to the online forms page to see

Try having them have their host run a FP SE server health check
