I have a user that can no longer open custom forms in the Organization Forms
Library (OFL) in Outlook 2003. His computer is a HP nw8440 laptop running
Windows XP Professional SP2. When he tries to open the majority of the forms
in the OFL, he gets the following error message:
Microsoft Office Outlook
The form you selected cannot be displayed.
When opening the other forms in the OFL, he gets the following error messages:
Script Error
The operation failed.

Line number 299 in the form contains the following:
Set RequestPage = Item.GetInspector.ModIfiedFormPages("Equipment")
He has tried clearing the forms cache within Outlook, as well as deleting
everything under C:\Documents and Settings\User.Name\Local
Settings\Application Data\Microsoft\FORMS. However, clearing the forms cache
does not resolve the problem. In addition to clearing the forms cache, we
have tried the following with no success:
- Tried running Outlook in safe mode
- Created a new profile in Outlook
- Created a new profile in Outlook under a different user account in Windows
- Uninstalled Office 2003 and installed Office 2007 (he needed to upgrade to
2007 anyway)
- Tried the registry key ForceFormReload under
HKLM\Software\Microsoft\Office\12.0\Outlook
- Tried running Office Diagnostics
Does anyone have any idea what could be causing this or have any suggestions
on how to fix it?
