Can't get Frame Template to work correctly

I've created a Introduction frame page using the Header, Footer and Contents
Frame Template. For some reason though the Header portion isn't showing up.
Could be that I didn't use the Frame template I thought I did.

What are my options for adding in the header frame to my existing frame
template? Should I remove the existing frame and then create a new Frame with
the Header, Footer and Contents template? I've tried just adding in a header
frame and can't seem to do it (No option that I see).

Lastly, am I thinking correctly in that when I create a Frame Template I
create just one (which I've done at my Intro or Index page) and then when I
publish my site it will use frames as long as my Index page is the first page
loaded? Or do I need a frame for each page of my site to insure frames are
used regardless of what page the site starts at?
