I have a user whose old Active Directory ID is showing up associated with her
email address display name that pops up in AutoComplete. For example, I want
to send an email to Smith, Jane. Her old AD ID is <SMITHJ>, and her new ID
is <WilsonJ>. SMITHJ was deleted from AD long ago. However, when I send an
email to her, "Smith, Jane <SMITHJ>" will appear in AutoComplete instead of
her new ID, which is Smith, Jane <WILSONJ>. We highlight the old entry,
delete it from AutoComplete cache, but it still shows up again after sending
a new email to her. This continues to happen each time we delete the cached
entry and send a new one.

If you highlight the old address and press shift delete then it should be
removed.
The other more definate way to remove it is to delete the whole cache file.
to do this search for and delete or rename all *.nk2
