Cannot add a Subject line in Outllook 2000 new message is missing.

Greetings: Using MS Outlook2000 - something happened to the configuration
and now when creating a "New Message" I only have the option to insert an
address in the "TO:" block. The "CC:" and "Subject" blocks do not appear at
all. I have tried the Help system, finding "settings and preferences," and
running the "Fix/Repair" utility but the problem still exists. TIA for any
assistance! - Chuck
