For the text you want a protected form, with form fields inserted into
cells of a table whose column width is fixed.
To get started with form fields, see

To fix the table column widths, click Table > AutoFit > Fixed Column
Width.
Images are a much harder problem, because there is no form field that
accepts them. You can allow the student to unprotect the form
temporarily, insert the image, and reprotect the form. If that's too
likely to let them mess up, you can look at the form_picture.zip file
that you can download from
http://jay-freedman.info -- but that's a
very advanced solution.
